What is the Cashless Claim Procedure at the Tata AIG Network Hospital in Lucknow?

The insured can get cashless treatment at the above-mentioned hospitals by following the below-mentioned cashless procedure.

For Planned Hospitalization

• Notify the insurer about the hospitalization five days before the treatment by submitting a duly filled cashless claim form
• If you receive a confirmation letter from the insurer, you’re eligible to receive cashless medical treatment at the network hospital
• After that, submit the confirmation letter and the health ID card to the hospital on the day of admission

Note: The confirmation letter is valid only for seven days from the date of issuance.

For Emergency Hospitalization

• Notify the insurer about the hospitalization within 24 hours
• On receiving the claim intimation, the insurer will generate a Claim Intimation/Reference Number
• Now, submit your insurance card, policy copy, ID and address proof and a duly filled KYC form to the insurer (If the claim amount is above INR 1 Lakh)
• The network hospital will submit the cashless claim request form to the company for authorization
• On approval, the insurer will send the authorization letter to the hospital

Note: The company/TPA can reject a pre-authorization if you don’t submit the relevant medical details. In case of rejection, the insurer will notify you via a letter, SMS on the registered mobile number and mail on the registered email ID.

What if the Tata AIG Network Hospital is Far From You?

If the network hospital isn’t near you, visit any non-network hospital and get treatment for your illness/injury. On discharge, pay all your bills and collect the payment receipt and all the necessary documents. After that, apply for a reimbursement claim by submitting the following documents to the insurer:

• Duly filled and signed claim form
• Hospital discharge card
• NEFT details
• A medical certificate signed by the attending doctor
• Original bills, receipts, pharmacy bills and investigation reports (if any) and pathological reports such as X-ray reports
• Insurance card or copy of the policy document
• Copy of the FIR / MLC (in case of an accidental claim)
• Duly filled KYC Form in case the claim amount exceeds INR 1 Lakh
