How to Get Cashless Treatment at the Tata AIG Network Hospital in Kolkata?

Receive cashless treatment at the above-mentioned Tata AIG Network Hospitals by following the procedure shown below:

Procedures to Follow for Planned Hospitalization

• Notify the insurer about the hospitalization at least five days before the hospitalization by submitting a duly filled cashless claim form to the insurer
• Once you receive the confirmation letter from the insurer, you’re eligible to receive cashless medical treatment at the network hospital
• On the day of admission, submit the confirmation letter and the health ID card to the hospital

Note: The confirmation letter is valid for seven days from the date of its issuance.

What to Do in Case of Emergency Hospitalization?

• Notify the insurer within 24 hours of hospitalization
• The insurer will generate a Claim Intimation/Reference Number
• After that, submit your insurance card, policy copy, ID and address proof and a duly filled KYC form to the insurer (If the claim amount is above INR 1 Lakh)
• The hospital will submit the cashless claim request form to the insurer
• Once the pre-authorization is provided by the insurer, you can receive cashless treatment at the network hospital

Note: The company/TPA reserves the right to reject pre-authorization if the insured doesn’t submit the relevant medical details. On rejection, the insurer will notify you via a letter, SMS on the registered mobile number and mail on the registered email ID.

What if the Tata AIG Network Hospital in Kolkata is Far from You?

If the network hospital isn’t near you, visit any nearby hospital and receive treatment for your illness/injury. Once you are discharged, pay all your bills and collect the payment receipt and other necessary documents and apply for a reimbursement claim. To do so, you may require the following documents:

• Duly filled and signed claim form
• Hospital discharge card
• NEFT details
• A medical certificate signed by the attending doctor
• Insurance card or copy of the policy document
• Copy of the FIR / MLC (in case of an accidental claim)
• Duly filled KYC Form in case the claim amount exceeds INR 1 Lakh
• Original bills, receipts, pharmacy bills and investigation reports (if any) and pathological reports such as X-ray reports
