How to File a Claim at the HDFC ERGO Network Hospital in Chandigarh?

You can receive cashless treatment at the above hospitals by following the below steps –

  • Visit the network hospital
  • Provide your health ID card and a valid photo ID proof at the hospital
  • After verifying your details, the hospital will send a cashless claim request to the insurance company
  • Later, the insurance company shall settle the claim with the hospital after your discharge
  • You’ll know about the claim approval via SMS or email

What to do if the Network Hospital isn’t Near You?

In such a case, you can go to a non-network hospital and receive treatment for the illness/injury. Later, file a reimbursement claim by following the below steps –

  • Pay the bills and collect the necessary documents before your discharge from the hospital
  • Upload the claim documents on the official website of the HDFC ERGO Health Insurance
  • Download the KYC or NEFT and Digital claim form from the official website of HDFC ERGO, fill it and submit the same.
  • After the submission of all the documents, HDFC ERGO’s doctors will review and send them to the insurer for claim processing
  • You’ll receive the claim approval through SMS or email within seven days from the date the last document is received by the company

Note – For any additional documents, the company will notify you via SMS and email. After getting all the claim documents, your claim request will be processed within seven days and the amount will be transferred to your linked bank account.

What Documents Do You Need for a Reimbursement Claim?

You need to submit the below documents to the insurer for a reimbursement claim –

  • Duly filled and signed claim form
  • Other relevant documents, if any
  • NEFT details for payment
  • Original discharge summary, the final bill with a detailed breakup, payment receipt, pharmacy invoices with prescriptions, investigation reports ( blood reports, X-Ray, etc.), implant sticker or invoice (for stent in angioplasty, lens cataract, etc.)
  • A cancelled cheque in the name of the proposer or passbook copy attested by the bank
  • Past treatment records, if any
  • In the case of an accident – Medico Legal Certificate (MLC) or FIR.
    In case the proposer dies, the nominee’s detail or the legal heir certificate if the nominee is a minor.
  • KYC documents if the claim amount is INR 1 lakh and above.
