Cashless Mediclaim Procedure

You can receive cashless medical treatment at Future Generali Network Hospitals in Pune by following the steps shown below –

1. Select any hospital from the Future Generali Network Hospital List in Pune
2. Visit the network hospital and show your FGH Health Insurance ID Card
3. Submit the duly filled pre-authorization form to the hospital

When the above procedure is done, the hospital shall submit the duly filled pre-authorization form to the TPA/insurer for further processing. Once your claim is approved by the insurer, you can receive cashless treatment at the network hospital without any hassle.

Reimbursement Claim Procedure

In case your cashless claim is not approved by the insurer, you can file a reimbursement claim by submitting the following documents to the insurer –

1. Duly filled claim form
2. Copy of policy certificate
3. Attending doctor’s report
4. Original admission/discharge card
5. Original bills along with payment receipts & prescription
6. Original X-ray reports, pathological and investigative reports, if any
