How to Apply for a Cashless Claim at Future Generali Network Hospital in Lucknow?
To avail of cashless medical treatment at Future Generali Network Hospital in Lucknow, follow the steps shown below –

- Show your FGH Health Insurance ID card at the reception along with your photo ID proof
- Submit a duly filled pre-authorization form to the hospital desk
Later, the hospital will send the pre-authorization form to the TPA/insurer for further processing. Once your cashless claim is approved by the insurer, you can receive cashless medical treatment for your illness or injury.
