How to Apply for a Cashless Claim?

To apply for a cashless claim, follow the steps shown below –

  1. Visit Future Generali network hospital in Gurugram
  2. Show your health ID card at the reception
  3. Submit the duly filled pre-authorization form to the hospital

Later, the network provider will send the pre-authorization form to the TPA/insurer for further processing. And if your claim is approved by the insurer, you can avail of the treatment at the network hospital without paying for the same.

What if Your Cashless Claim is Rejected?

If your cashless claim is not approved, you can file a reimbursement claim by submitting the below-mentioned documents to the insurer –

  1. Duly filled Claim Form
  2. Copy of Policy Certificate
  3. Attending Doctor’s report
  4. Original Admission/Discharge card
  5. Original bills along with payment receipts & prescription
  6. Original X-ray reports, pathological and investigative reports, if any
