What is the Future Generali Health Insurance Cashless Claim Process?

In a cashless claim, the Third Party Administrator (TPA) will authorize the network hospital for the treatment of your illness/injury. To file a claim, you need to follow the steps shown below:

  • Visit the network hospital
  • Show your health ID card along with an ID proof
  • Submit the duly filled pre-authorization form to the hospital
  • Once the pre-authorization is approved, the TPA shall pay your bills directly to the hospital.

How to Obtain Pre-authorization for Cashless Claims?

You can get the pre-authorization form from the TPA office or download the same from its website. One should mention the following details in the pre-authorization form.

  • Name of the insured/patient
  • Name of the hospital where treatment is required
  • Nature of illness/disease
  • Monetary limit above which the insured/patient needs to pay
  • Submit the pre-authorization letter along with the health ID card to the admission counter at the network hospital.

How to Apply for a Claim if the Future Generali Network Hospital is Far from You?

In such a case, you can receive treatment at a non-network hospital and pay your bills on discharge. Later, apply for reimbursement by following the steps shown below:

  • Intimate the TPA within a few days from the time of admission with details of TPA card number, nature of illness, name & address of the Hospital, Nursing Home, Clinic, attending Doctor, Bed Number, etc.
  • After that, submit the duly filled claim form to the nearest TPA office along with the required documents.

List of the Documents for a Reimbursement Claim

  • Duly filled and signed claim form
  • Duly filled and signed Medical Certificate form stamped by the treating doctor
  • Original discharge card or summary from the hospital/nursing home
    Doctor’s consultation reports/history.
  • Hospitalization and other medical bills, receipts
  • Cash Memos from the hospital/pharmacies with a prescription
  • Investigation reports
  • Medico-Legal Certificate (MLC) in case of an accident

Note: The above list is indicative, so you might need to submit additional documents as per your claim. The insurer shall reimburse the expenses relating to hospitalization. However, non-medical expenses won’t get reimbursed.

Procedure to Report a Health Claim

You can intimate the insurer about the claim by following the steps shown below:

  • Visit the official website of Future Generali India Insurance
  • Select the ‘Claim’ option
  • Click on the ‘Report A Health Claim’
  • Enter the Patient Name, Health Id Card Number, Mobile Number and Email ID
  • Enter Hospital Name, Treating Doctor’s Contact Number, Address
  • Select State, City and enter the Pin Code
  • Enter the date of admission and discharge
  • Enter the approximate medical expenses
  • Name of the illness/disease
  • Upload the necessary documents
  • Click on the ‘Submit’ button
