Step by Step Cashless Claim Process

To get cashless medical treatment at Edelweiss General Insurance Network Hospitals in Chennai, follow the steps shown below:

  1. Visit any of the above-mentioned network hospitals in Chennai
  2. Show your health card and a valid ID proof at the hospital reception
  3. Submit the duly filled and signed pre-authorization form to the hospital

The network provider will send your pre-authorization form to the insurer, and when your claim request is approved, you’ll be notified about the same.

How to Apply for Reimbursement?

Unfortunately, if your cashless claim request is rejected, don’t worry, you can file a reimbursement claim by submitting the following documents to the insurer –

  1. Duly filled and signed Claim Form
  2. Copy of health ID card or policy document
  3. Discharge summary
  4. Hospital bill with break up of each item
  5. Payment receipt of the hospital bill with the receipt number
  6. Consultation letter and subsequent prescriptions from the attending medical practitioner
  7. Bills, payment receipts and reports for investigation
  8. Surgeon certificate stating the nature of operation performed, along with surgeon’s bills and receipts, if required
  9. Fees of doctors, consultants, specialist’s, anesthetist bills, receipts and certificate regarding the treatment
  10. Pharmacy bills and receipts with prescriptions
  11. Original invoice/bills for implants with original payment receipts
  12. Copy of the First Information Report (FIR) or Medico-Legal Certificate (MLC), if applicable
