Step by Step Procedure for Cashless Treatment at Edelweiss Network Hospitals in Lucknow
You can receive cashless medical treatment at Edelweiss Network Hospital if you follow the steps shown below:
- Visit any of the above-mentioned network hospitals in Lucknow
- Show your health ID card at the hospital reception
- Submit the duly filled and signed pre-authorization form to the network hospital
When you are done with the above, the network hospital will send the pre-authorization form to the insurance company. Once pre-authorization for cashless medical treatment is approved, you can receive cashless medical treatment at the network hospital within 10 days of the pre-authorization.
What if Pre-authorization is Rejected?
In case your pre-authorization for the cashless claim is rejected, file a reimbursement claim by submitting the following documents to the insurer –
- Duly filled and signed claim form
- Copy of health ID card or policy document
- Original discharge summary from the hospital
- Original consolidated hospital bill with the break up of each item
- Original bills, payment receipts and reports for investigation
- Attending doctors, consultants, specialist’s, anesthetist bills and receipts and certificates regarding the treatment of the illness/injury
- Surgeon certificate stating the nature of operation performed, surgeon’s bills and receipts, if applicable
- Original medicine bills and receipts with prescriptions from the attending doctor
- Original invoice/bills for implants with original payment receipts, if applicable
- Original payment receipt of the hospital bill with the receipt number
- First consultation letter and subsequent prescriptions
- Copy of the First Information Report (FIR) or Medico-Legal Certificate (MLC), if applicable
Intimation of Claim
You can notify the insurer about the hospitalization by calling the helpline number/sending an email to support@edelweissinsurance.com. But when to intimate the insurer?
- Planned Hospitalization – Notify the insurer 72 hours before the scheduled admission.
- Emergency Hospitalization – Notify the insurer within 48 hours of admission/discharge.
