1. Notify the insurer about the hospitalization
2. Download the pre-authorization form
3. Submit the duly filled pre-authorization form to the hospital’s Insurance/TPA desk

The network hospital will fax the completed pre-authorization form to the insurer at 1800-200-6677.

Reimbursement Claim Procedure

In case your cashless claim isn’t approved, you can apply for reimbursement by following some instructions. Important Notes for Cashless & Reimbursement Claims

1. Notify the insurer within 24 hours of the admission in case of emergency hospitalization
2. Notify the insurer 2 days before the planned hospitalization
3. Intimate about the hospitalization by calling 1800-102-6655, 1800-102-4488 (toll-free) or sending an email to customerfirst@careinsurance.com
4. 5. Documents required for reimbursement claim are – insured’s ID proof, referral letter from the attending doctor, payment receipts for pharmacy along with prescription, hospital main bill, diagnostic report, FIR or Post Mortem report, if applicable. The insurance company may ask for further documents if required.
