Cashless Treatment Procedure at Care Network Hospitals in Noida

In case you are sick or injured, you can get cashless treatment at Care network hospitals in Noida by following the steps below –

  1. Intimate the insurer about the hospitalization
  2. Download the pre-authorization form from the insurance company’s official website
  3. Submit the duly filled pre-authorization form to the hospital’s Insurance/TPA desk

After that, the network hospital will fax the duly filled pre-authorization form to the insurer at 1800-200-6677. The insurer will notify you and the network hospital when the claim gets approved.

Reimbursement Claim Procedure

If your cashless claim is rejected by the insurer, you can go for reimbursement by paying the costs upfront and get that reimbursed from Care Health after discharge. The steps for reimbursement claims are shown below.

  1. Notify the insurer about the hospitalization
  2. Pay your hospital bills at the time of discharge
  3. Download the claim form from the insurance company’s official website
  4. Submit the duly filled claim form and the required documents to the following address – Care Health Insurance Limited(formerly known as Religare Health Insurance Company Limited), Unit No. 604 – 607, 6th Floor, Tower C, Unitech Cyber Park, Sector-39, Gurugram-122001 (Haryana).

You will get a notification on your registered mobile or email when the insurer approves the claim.

Documentation for Reimbursement Claims

You need to submit the following list of documents to get reimbursement from the insurer –

  1. Copy of ID proof – Aadhaar Card, Voter ID Card, PAN Card, Driving Licence
  2. Referral letter from the attending doctor
  3. Pharmacy/chemists bills along with doctor’s prescription
  4. Diagnostic tests or consultation reports like – X-ray, blood test reports, etc.
  5. Discharge card/summary
  6. Indoor case papers
  7. First Information Report (FIR), if required
  8. Post mortem report, if required

Note – The insurance company may ask for additional documents, as the case may be.

The time limit for claim intimation is 24 hours from the date/time of your admission for emergency hospitalization, and 48 hours before a planned hospitalization.
