Cashless Process at Care Network Hospitals

To get cashless treatment at Care network hospitals in Mumbai, follow the steps shown below –

1. Intimate the insurer
2. Download the pre-authorization form from the insurer’s official website
3. Submit the duly filled pre-authorization form to the hospital’s Insurance/TPA desk

Later, the network hospital will fax the duly filled pre-authorization form to the insurer at 1800-200-6677.

Reimbursement Claim Process

If your cashless claim is rejected by the insurer, you can seek reimbursement of your medical expenses post-discharge from the hospital. To get reimbursement, follow the steps shown below:

1. Notify the insurer about the hospitalization
2. Pay hospital bills at the time of discharge
3. Download the claim form from the insurer’s official website
4. Submit the duly filled claim form plus the required documents to – Care Health Insurance Limited(formerly known as Religare Health Insurance Company Limited), Unit No. 604 – 607, 6th Floor, Tower C, Unitech Cyber Park, Sector-39, Gurugram-122001 (Haryana).

You will be notified via SMS or email if your reimbursement claim is approved by the insurer. The incurred medical expenses shall be reimbursed to your bank account through an online transfer.

Reimbursement Documentation 

Here’s a list of documents you need to submit to get reimbursement from the insurer –

1. Copy of ID proof – Aadhaar Card, Voter ID Card, PAN Card, Driving Licence
2. Referral letter (from the medical practitioner)
3. Pharmacy/chemists bills along with the prescription
4. Diagnostic tests or consultation reports – X-ray, blood test reports, etc.
5. Discharge card/summary
6. Indoor case papers
7. First Information Report (FIR), if required
8. Post mortem report, if required

The insurance company may ask for any other documents for further investigation, if necessary.
