How to Intimate Care Health Insurance About the Hospitalization?

You can notify the insurer about the hospital admission through the following means –

Call at – 1800-102-6655 or 1800-102-4488 (toll free number)

Send an email to – customerfirst@careinsurance.com

There’s a time limit for claim initiation –

For Emergency Hospitalization – You need to intimate the insurer within 24 hours of the admission.

For Planned hospitalization – You need to intimate the insurer 2 days before you get admitted to the Care Network Hospital in Delhi.

Cashless Claim Procedure at Care Network Hospitals in Delhi

You can get health insurance claims conveniently by following the steps shown below –

1. Intimate the Insurer
2. Download the Pre-Authorization form from Care Health’s official website
3. Submit the duly filled and signed Pre-Authorization form to the hospital’s Insurance/TPA desk

The network hospital will fax the completed pre-authorization form to the insurer at 1800-200-6677. If your claim is approved, you and the network hospital get an SMS or email notification regarding the same.

Reimbursement  Claim Procedure at Care Network Hospitals in Delhi

You can seek reimbursement of incurred medical expenses if your cashless claim is rejected by the insurer. You will need to pay for the expenses upfront and get that reimbursed from Care after getting discharged from the hospital. In reimbursement claims, payments are made through online bank transfers only. To apply for a reimbursement claim, you need to follow the steps shown below –

1. Intimate the Insurer
2. Download the claim form from Car Health official website
3. Submit the duly filled form to the insurer along with the necessary documents

List of Documents Required for Reimbursement  Claim

Submit the following documents mentioned below for a speedy claim disbursal –

1. Indoor Case Papers – (doctor notes, nursing notes, patient progress details and patient condition summary from the date of admission till discharge from the hospital)
2. Discharge Summary – (This report includes – admission date, discharge date, diagnosis, line of treatment and further advice on discharge)
3. Payment Receipts – (All bills, investigation reports or any other procedure you have undergone)
4. Consultation Papers (written prescription of the attending medical practitioner)
5. Canceled cheque
6. First Information Report (FIR), if applicable
7. Post mortem report, if conducted

You need to submit the duly filled claim form plus the above-mentioned documents to the following address –
