Step-by-Step Claim Procedure at Care Network Hospitals in Chennai
If you want to receive cashless medical treatment at Care network hospitals in Chennai, you need to follow the steps shown below –
- Notify the insurer about the hospitalization by calling the toll-free number – 1800-102-4488 or send an email to customerfirst@careinsurance.com.
- Download the pre-authorization form by visiting the insurer’s official website
- Fill the pre-authorization form and submit it to the hospital’s Insurance/TPA desk
After verifying the pre-authorization form, the hospital will fax it to the insurer at 1800-200-6677.
Note – You need to notify the insurer about the hospitalization within 24 hours of your admission in case of emergency hospitalization. Whereas for a planned hospitalization, the intimation period is up to 48 hours before your admission.
What If Your Cashless Claim is Rejected by Care Health?
You can apply for reimbursement if your cashless claim is not approved. So, you can pay the hospital bills first and get reimbursement post-discharge from the hospital, For reimbursement, you need to follow the steps shown below –
- Notify the insurer
- Download the claim form
- Fill the claim form and send it to – Care Health Insurance Limited(formerly known as Religare Health Insurance Company Limited), Unit No. 604 – 607, 6th Floor, Tower C, Unitech Cyber Park, Sector-39, Gurugram-122001 (Haryana) along with the required documents.
When your claim request is approved, you will receive an SMS or email on your registered mobile or email ID.
Documents Required for Reimbursement Claim
Below is a list of documents that you need to submit to the insurer to seek reimbursement –
- Copy of a valid photo ID proof – Aadhaar Card, Voter ID Card, PAN Card, Driving Licence
- Referral letter from the attending medical practitioner
- Pharmacy bills along with prescription
- Diagnostic tests or consultation report
- Discharge card/summary (in original)
- Indoor case papers
- First Information Report (FIR), if applicable
- Post mortem report, if conducted
