Cashless Claim Procedure

To get cashless treatment at Care network hospitals in Ahmedabad, you need to follow the steps shown below –

1. Notify the insurer about the hospitalization
2. Download the pre-authorization form by visiting Care Health’s official website
3. Submit the duly filled pre-authorization form to the hospital’s Insurance/TPA desk

The network hospital will fax the pre-authorization form to the insurer at 1800-200-6677. And when your cashless claim is approved, the insurer will notify you and the network hospital.

Reimbursement Claim Procedure

In case your cashless claim isn’t approved, you can seek reimbursement for your medical expenses from the insurance company. You need to first pay your hospital bills and after discharge from the hospital, you can apply for reimbursement by following the steps shown below –

1. Notify the insurer about the hospitalization
2. Download the claim form by visiting Care Health’s official website
3. Submit the duly filled claim form and the necessary documents to – Care Health Insurance Limited(formerly known as Religare Health Insurance Company Limited), Unit No. 604 – 607, 6th Floor, Tower C, Unitech Cyber Park, Sector-39, Gurugram-122001 (Haryana).

When your reimbursement claim is approved, you will get a notification on your mobile or email. The approved claim amount shall be reimbursed to your bank account through an online transfer.

List of Documents Required for Reimbursement Claims

The insurer will require the following documents to process your reimbursement claim –

1. Copy of ID proof – Aadhaar Card, Voter ID Card, PAN Card, Driving Licence
2. Referral letter from the attending medical practitioner
3. Pharmacy/chemists bills along with the prescription of the attending medical practitioner
4. Diagnostic tests or consultation reports such as – X-ray, blood test reports, etc.
5. Discharge card/summary
6. Indoor case papers
7. First Information Report (FIR), if required
8. Post mortem report, if required

Note – The insurance company may ask for any other documents for further investigation if required.

2. For emergency hospitalization, intimate within 24 hours of your admission
3. For planned hospitalization, intimate 48 hours before your admission
