Intimate the Insurer About the Hospitalization
You can contact the insurer by calling the toll-free number 1800-103-2292 or sending an email to claims@bhartiaxa.com, or you can log in to Bharti AXA’s official website and intimate the insurer. You need to share the following details with the insurer –
- Name of the insured
- Policy number
- Contact details
- Nature of the disease
- Name and address of the hospital
- Date of hospitalization
Note – The claim notice must be sent within 48 hours of admission or discharge from the hospital, whichever is earlier.
Cashless Claim Procedure at Bharti AXA Network Hospitals in Hyderabad
- Show your Health ID card at the network hospital
- Upon admission to the network hospital, whether for a planned or emergency hospitalization, you need to fill a pre-authorization form and submit it to the hospital authority
- The hospital will fax or mail the duly filled pre-authorization form to the TPA or insurer
- The insurance company or TPA will notify you or the hospital about the approval/rejection of the claim within 6 hours (emergency hospitalization) and 48 hours (planned hospitalization) of submission of the request.
Reimbursement Claim Procedure
If your cashless claim is rejected, you can receive reimbursement later. To apply for a reimbursement claim, intimate the insurer within seven days from the date of discharge. However, you need to submit the following documents to the insurer along with the duly filled reimbursement claim form –
- Copy of Policy Certificate
- Claimant’s Statement
- Final Hospital Bill
- Admission notes
- Discharge summary
- Test reports (Including past medical records, if any)
- Treating Doctor’s Certificate
When the requisite documents are submitted, the insurer will settle the claim within 30 days.
