Cashless Claim Process at Bajaj Allianz Network Hospitals in Lucknow

Step 1: Get admission to one of the nearest or convenient Bajaj Allianz Network Hospital in Lucknow.

Step 2:  After the completion of the hospitalization process, Bajaj Allianz will verify your details. In case of planned hospitalization, fill the pre-authorization form and submit it to the network hospital. Whereas in emergency hospitalization, you will need to show your Bajaj Allianz Health Card along with the pre-authorization form.

Step 3: Once verification is completed, the network hospital will send the duly filled pre-authorization form to Bajaj Allianz Health Administration Team (HAT).

Step 4: To approve your cashless claim request, Bajaj Allianz will verify your pre-authorization form and other details.

Post successful approval, Bajaj Allianz will settle the medical bills as per the terms and conditions of your health insurance plan.

Important Note: To get the cashless claim, you should inform Bajaj Allianz at least 48 hours before a planned hospitalization and within 24 hours of an emergency admission. 

What to Do if Your Cashless Claim Gets Rejected at Bajaj Allianz Network Hospitals in Lucknow?

You may face rejection and it’s important not to panic in such a situation. The reason: you can file a reimbursement claim after getting discharged from the hospital. To do this, you will need to submit the following documents.

1. Hospitalization Claim form duly filled and signed by the insured

4. Copy of Invoice/Stickers/barcode in case of implants
5. A first consultation letter from the doctor
6. KYC form
7. Original hospital bill with detailed cost breakup
8. Original paid receipts
9. Filled and signed NEFT form by Policyholder/Proposer

To evaluate your reimbursement claim request, Bajaj Allianz will do a customary verification of the required documents. Post-approval, the payment will be transferred to your account via ECS within 7 working days.
