Aditya Birla Network Hospitals Cashless Claim

To make a cashless claim under Aditya Birla Health Insurance Plan, you need to follow the steps shown below –

1. Show the health insurance card at the network hospital information desk or your policy number
2. Provide valid ID proof for verification, such as Passport, PAN Card, etc.
3. When the verification process completes, the network hospital will send a pre-authorization form to Aditya Birla
4. The insurer will review the pre-authorization form and approve the claim only upon successful verification
5. When the pre-authorization form is approved, the same will be communicated to the network hospital

Note – You can also send the pre-authorization form to the insurer within 48 hours (for emergency hospitalization) or 3 days before the hospital admission (for planned hospitalization).

Aditya Birla Reimbursement Claim Procedure

If your cashless claim is not approved or you are admitted to a non-network hospital, you can seek reimbursement. To raise a reimbursement claim, first notify your insurer about the hospitalization.

For Planned Hospitalization – You can notify the insurer about the hospital admission for the injury or illness treatment 3 days before you get admitted for the same.

For Emergency Hospitalization – Notify the insurer within 48 hours of admission if it is an emergency hospitalization.

After that, you need to pay your hospital bills and collect the payment receipt and the following documents –

1. Duly filled Claim form Part A
2. Duly filled Claim form Part B
3. Copy of Health card or Policy
4. KYC documents
5. Copy of Hospital Registration Certificate
6. Discharge card, daycare summary, transfer summary or death summary in original
7. Final hospital bill with break up in original
8. Original deposit and final payment receipts/invoice
9. Original payment receipt for charges billed outside the main hospital bill
10. Implant stickers and invoice for all implants (in case of surgeries)
11. Letter from the treating medical practitioner stating complaints, duration, past medical history and treatment details
12. Previous consultation papers related to the current ailment
13. Original diagnostic reports including imaging and laboratory tests along with prescriptions and bills.
14. Pharmacy/Medical bills along with the prescription
15. Medico-Legal Certificate MLC/FIR copy (in case of an accident)
16. Letter from the treating medical practitioner stating the details of accident/trauma and influence of alcohol/intoxication (in case of an accident)
17. Pre and post-operative imaging reports (in case of an accident)
