Aditya Birla Cashless Claim Procedure

Select any network hospital of Aditya Birla from the above list and follow the steps shown below –

1. Show your Aditya Birla Health Insurance Cashless Card or at the hospital reception or show your policy number
2. Give your valid ID proof such as Passport, PAN card, Voter’s ID, etc. for identity verification
3. After that, the hospital sends a pre-authorization form to Aditya Birla

To speed up the pre-authorization process, the insured can also intimate the insurer by filing the pre-authorization form online. If you are admitted to the network hospital in case of an emergency, the request form can be submitted within 48 hours of hospitalization. Whereas, if there’s a planned hospitalization, the request form should be submitted three days before your admission to an Aditya Birla Network Hospital in Ghaziabad. When the company receives the insured’s pre-authorization request, it will review the same and notify its decision of approval to the network hospital and insured within 2 hours via SMS or email.

Note – The SMS or email for pre-authorization confirmation or rejection shall be received within 2 hours only if the form is received during the usual business hours.

Reimbursement Process at Aditya Birla Network Hospitals in Ghaziabad

6. Original discharge card, daycare summary, transfer summary or death Summary
7. Original final hospital bill with break up along with all the original deposit and final payment receipts.
8. Original invoice along with the payment receipt for charges that are billed outside the hospital main bill
9. Implant stickers & invoices used during the surgeries
10. Letter from the treating doctor stating complaints, duration, medical history and treatment details
11. Previous consultation papers for the current ailment
12. Original diagnostic reports with the prescription and bills
13. Pharmacy/Medical bills along with the Medical Practitioner prescription

In Case of Hospitalization due to Accident

1. Medico-Legal Certificate (MLC) or First Investigation Report (FIR) copy
2. Letter of the treating medical practitioner stating the details of an accident or trauma and influence of alcohol and any intoxicants
3. Pre and post-operative imaging reports
