Steps to Raise Cashless Claim at Aditya Birla Network Hospitals in Delhi

  1. Visit any of the network hospitals in Delhi
  2. Show your Aditya Birla Health Insurance Cashless Card or give your policy number
  3. Show a valid ID proof – Aadhaar Card, Voter ID Card, Driving Licence, etc. for verification
  4. After that, the hospital will send a pre-authorization form to the insurer
  5. The insurer will review the pre-authorization form and communicate its decision to the network hospital

Note  – For a speedy approval of the pre-authorization form, you can intimate the insurer by yourself. If it is an emergency hospitalization, send the pre-authorization form within  48 hours of the admission. Otherwise, for a planned hospitalization, you need to submit the form 3 days before hospitalization.

Steps to Raise Reimbursement Claim at Aditya Birla Network Hospitals & Non-Network Hospitals in Delhi

  1. Notify the insurer about the hospitalization immediately on the official website, at care.healthinsurance@adityabirlacapital.com, the Whatsapp at +91 8828800035.
  2. If it is an emergency hospitalization, you need to notify the insurer within 48 hours of the admission
  3.  If it is a planned hospitalization, you need to notify the insurer three days before the admission
  4. After that, collect all the necessary documents in original from the hospital and submit them to – Aditya Birla Health Insurance Co. Limited 5th floor, Modi Business Centre, Kasarvadavali, Mumbai, Thane West 400 615 within 15 days of discharge from the hospital
  5. When the insurer receives the documents, it will review them and approve the claim accordingly

  5. Original invoice and payment receipt for the charges which are billed outside the hospital main bill

  6. Implant stickers and invoices that are used during the surgeries

  7. Letter from the treating medical practitioner stating the complaints, duration, past medical history and received treatment details

  8. All the previous medical consultation papers related to the current ailment

  9. Original diagnostic reports including imaging and laboratory reports along with prescription and bills

  10. Pharmacy or medical bills along with the doctor prescription

  2. Claim form Part B (Duly completed by the Hospital)

Only in the case of an accident, the insurer requires the following additional documents that are as follows -

  1. Medico Legal Certificate (MLC)

  2. First Investigation Report (FIR)

  3. Letter from the treating medical practitioner stating the details of the accident or trauma and if there is any influence of alcohol/intoxication

  4. Pre and post-operative imaging reports

Note – The above list of documents may vary depending on the insured’s case. Aditya Birla may ask for additional documents in case of information deficiency or further investigations.
