How to Apply for a Cashless Claim at the Aditya Birla Network Hospitals in Bhopal?

To apply for a cashless claim, you need to go through the following steps:

Step 1: Notify the insurer about the claim
Step 2: Show your health ID card at the network hospital along with a valid ID proof
Step 3: Collect the pre-authorization form from the helpdesk, fill in the details and submit it to the hospital
Step 4: The hospital will send your duly filled pre-authorization form to the insurer for approval

Pre-authorization for Planned Hospitalization

In case of planned hospitalization, notify the insurer 72 hours before the date of admission. The pre-authorization request should be accompanied by the following documents:

• Health ID card
• KYC documents
• Policy number
• Name of the policyholder
• Name and address of the insured person for whom the claim request is filed
• Nature of the illness/injury and the required treatment/surgery
• Name and address of the attending Medical Practitioner
• Name of the hospital where treatment/surgery is proposed to be taken

Pre-authorization for Emergency Hospitalization

In case of an emergency, notify the insurer within 24 hours of the admission. You need to accompany the following details when making a pre-authorization request:

• Health ID card
• KYC documents
• Policy number
• Name of the policyholder
• Name and address of the insured person for whom the claim request is filed
• Nature of the illness/injury and the required treatment/surgery
• Name and address of the attending Medical Practitioner
• Name of the hospital where treatment/surgery is about to happen
• Date of admission

What if the Network Hospital is Far from Your Residence or the Place of Accident?

In such a case, apply for a reimbursement claim by following the steps shown below:

• Notify Aditya Birla Health Insurance about the hospitalization within 48 hours of admission/discharge
• Visit any non-network hospital and received treatment for the covered illness/injury
• Upon discharge pay your bills and collect the necessary documents

List of Documents Required for Filing a Reimbursement Claim

• Duly filled claim form Part A
• Duly filled claim form Part B
• Copy of the health ID card or policy document
• KYC documents
• Copy of the hospital registration certificate
• Final hospital bill
• Deposit and final payment receipts
• Implant stickers and invoices if surgery is performed
• Original discharge card, daycare summary, hospital transfer summary or death summary
• Invoices with the payment receipt for charges that are billed outside the hospital
• Letter from the attending medical practitioner stating complaints, duration, medical history and treatment details
• Past consultation reports for the current ailment/injury for which you are admitted
• Original diagnostic reports including imaging and laboratory tests along with prescription and bills
• Pharmacy or medical bills with doctor’s prescription
• Medico-Legal Certificate (MLC) or FIR copy in case of an accident
• Letter from the medical practitioner stating the details of accident/trauma and influence of alcohol/intoxication (required in case of an accident)
• Pre and post-operative imaging reports (required in case of an accident)
