Cashless Claim Procedure at Aditya Birla Network Hospitals

You can make a successful cashless claim at Aditya Birla Network Hospitals in Bengaluru by following the steps shown below –

  1. Show your Aditya Birla Health Insurance Cashless Card or provide the policy number
  2. After that, show your valid ID proof such as Aadhaar Card, Voter ID Card, Passport, PAN Card for verification

After the verification process completes at the network hospital, the hospital administration will send a cashless hospitalization request form (Pre-authorization Form) to the insurer.

You can speed up your claim process by sending a cashless hospitalization request form to the insurer. If it is a medical emergency, you need to submit the pre-authorization form within 48 hours from the time of admission to the hospital. Whereas, if it is a planned hospitalization, the pre-authorization form needs to be submitted three days before admission.

When the pre-authorization form is received by the insurer, it will intimate its decision to the network hospital. You will also receive an SMS on your registered mobile number or your email address regarding the pre-authorization form.

Reimbursement Process of Aditya Birla Health Insurance

If your cashless claim is rejected due to any reason, don’t worry because you can still get compensation for your medical expenses by raising reimbursement claims with Aditya Birla Health Insurance. Follow the steps shown below to raise a reimbursement claim –

Intimate the insurer about the hospitalization immediately

  1. For Emergency Hospitalization – You can notify the insurer within 48 hours of the hospitalization
  2. For Planned Hospitalization – You need to notify the insurer 3 days before the hospitalization

Note – You can notify the insurer about the claim by visiting the website and logging in to your account, sending an email to the insurance company’s official email address or calling the toll-free number.

Collect the below documentation in original from the hospital and submit them to the insurer’s office address within 15 days of discharge from the hospital

  1. Duly filled Claim form Part A by the insured
  2. Duly filled Claim form Part B by the Hospital
  3. Copy of your health card or policy bond
  4. KYC documents – Photo ID, Address and age proof
  5. Copy of the Hospital Registration Certificate
  6. Original discharge card, daycare summary, transfer summary or death summary, whichever is your case
  7. Original main hospital bill with break up along with the original deposit and final payment receipts
  8. Original invoice with the payment receipt for the charges which are billed outside the main hospital bill
  9. Implant stickers and invoices, if any implants used during the surgeries
  10. Letter of the treating medical practitioner stating the complaints, duration, past medical history and treatment details of the insured
  11. All the previous medical consultation papers for the current ailment/illness
  12. All the original diagnostic reports that include imaging and laboratory test results plus the prescription and bills receipt
  13. Pharmacy/Medical bills with the Medical Practitioner prescription

If it is a case of an accident, besides the above-mentioned documents the insured need to submit the following as well –

  1. Medico-Legal Certificate (MLC) or First Investigation Report (FIR) copy
  2. Letter of the treating medical practitioner stating the details of the accident or trauma and the influence of alcohol/intoxication
  3. Pre and post-operative imaging reports
