How to Get Cashless Treatment at the Aditya Birla Network Hospital in Guwahati?
Follow the below steps to apply for a cashless claim at the above hospitals in Guwahati.
- Go to the chosen network hospital and show your health ID card or policy document at the reception
- Submit your ID proof – Aadhaar Card, Voter ID Card, Passport, etc. at the hospital reception
- Get your pre-authorization form from the insurance help desk, fill the form and submit it at reception. You need to submit the duly filled pre-authorization form within 48 hours of your emergency admission and three days before a planned hospitalization.
- The network hospital will submit your duly filled pre-authorization form to the insurance company to process your claim
- And after reviewing your pre-authorization request, the company will communicate its decision to you and the hospital through SMS or email
What if the Aditya Birla Network Hospital isn’t Nearby?
If that’s the case, you can visit a non-network hospital and receive treatment there. And on your discharge, pay the bills and collect the necessary documents from the hospital. Later, apply for a reimbursement claim by following the below steps –
- Notify the insurer about the claim within 48 hours of emergency admission and three days before a planned hospitalization
- Submit the required documents to the insurer at the Aditya Birla branch office
- And after receiving the claim request, the insurer will review it and starts the claim processing
The List of Documents for a Reimbursement Claim
To file a reimbursement claim, you need to submit the following documents to the insurer within 15 days of your discharge.
- Claim form Part A, Claim form Part B, Health card or Policy copy, KYC proof
- Hospital registration certificate, discharge card, daycare summary, transfer summary or death summary, as per the claim
- Hospital main bill, invoice, payment receipt for the charges incurred outside the hospital, implant stickers/invoice, pharmacy bills
- Medico-Legal Certificate (MLC), First Investigation Report (FIR) copy, details of the accident, trauma, alcohol and other intoxication intakes, pre and post-operative imaging reports for an accidental hospitalization
