Documents Required While Buying Health Insurance

While buying health insurance, the insurer would want you to submit the documents authenticating your identity, age, address, and medical reports in certain cases. Let’s look at such documents below.

Identity Proof – Aadhaar Card/Voter ID/Driving Licence/Passport/PAN Card

Age Proof – Passport/Birth Certificate/10th or 12th Mark Sheet/Aadhaar Card/Driving License/Voter ID/PAN Card

Address Proof – Aadhaar Card/Electricity Bill/Telephone Bill/Passport/Voter ID/Driving License/Ration Card

In addition, some health insurance companies may ask you to submit medical reports and undergo certain tests too.

What to Submit When Filing Cashless Health Insurance Claims?

With cashless health insurance claims, you don’t need to pay for the medical expenses during hospitalization. The health insurance company will pay the bill to the network hospital on your behalf. However, these claims are possible only at the network hospital of the insurer. You can find the list of network hospitals on the official website of the health insurance company. For a hassle-free cashless claim, you need to show your health ID card and submit the following documents.

Pre-authorization Form – This form is available at the desk of the network hospital. You can fill it and submit it to the concerned official for further processing. It serves as an intimation to the insurance company about your claim. Now, the timing for the submission of this document will vary on the type of hospitalization. You would thus need to submit the form 48-72 hours before the scheduled hospitalization and within 24 hours of emergency admission.

Besides, you need to submit any of these documents – PAN Card, Voter ID, Aadhaar Card or Passport – for further verification.

After all these, the network hospital will send the pre-authorization form and other details to the insurer. The insurance company will then scrutinize these and approve the cashless claim upon successful verification.

Documents to Submit When Filing a Reimbursement Claim

In case the network hospital is far away from your office or residence, you can file a reimbursement claim. Here, you will need to pay upfront and get all that reimbursed from the company by submitting the following documents after discharge.

  1. Original claim form duly filled and signed by you (mention your policy name, policy number and other details correctly)
  2. Valid identity proof – PAN Card/Passport/Driving License/Voter ID
  3. Doctor’s prescription advising treatment in a hospital, medicines, diagnostic tests, consultation, etc.
  4. Original pharmacy bills
  5. First Information Report (If the case warrants the same)
  6. Indoor case papers

The insurance company will verify the submitted documents and other details and approve your reimbursement claim only after successful authentication. You will get the money in your bank account a few days after approval.

Note – Regardless of the type of claim you choose, the insurer won’t pay for any inadmissible expenses. You can check such expenses under the list of exclusions in your policy document.
